Clinical Studies on Acute Meningitides in Infants and Children


Abstract
73 cases of acute purulent meningitis and 97 cases of aseptic meningits were clinically observed at the department of pediatrics, Korea Univ. Medical School for 9 years 10 months during the years 1969 through 1978.
The followings were obtained as results.
1) The total cases of acute meningitis were 170, among which the male was 117(7016), the female, 53 cases(300o). Thus the male to female ratio was aproximately 3 : 1.
2) According to the age distribution, the highest incidence in acute purulent meningitis was in the age between 1-12 months after birth(49.9566) while in aseptic meningitis was in the age of pre and post school child(47.4%).
3) Seasonal distributions were revealed in two different ways. Acute purulent meningitis raised the highest in winter(30.10o), besides aseptic meningitis did in summer (40. 2%).
4) The proceeding or associated diseases in acute purulent meningitis were U.R.I. (28.8%) followed by pneumonia (8.251o) and those in aseptic meningitis were mumps(18.0/6) followed by U.R.I.(16.550).
5) The most frequent symptoms on admission were fever, vomiting and convulsion, while that of physical sign was neck stiffness(over 80%) in both meningitides.
6) The WBC count in peripheral blood in acute purulent meningits was measured in 41.1
between 10,000-20,000 cmm and in aseptic meningitis in 46.4% under 10,000 cmm.
7) The finding of C.S.F. revealed remarkably increased cell count and PMN cells were predominant in acute purulent meningitis (500-5, 000/mms, 52.0/6) in addition that lymphocytes were predominant in aseptic meningitis outstandingly (over 75% of lymphocyte, 62.900).
8) Sugar in C.S.F. was moderately decreased in acute purulent meningitis as compared with the case in aseptic meningitis but protein value was remarkably increased in acute purulent meningitis. No special changing of chloride was observed in both.
9) Among the 73 cases of purulent meningitis, pneumococcus, meningcoccus and staphylococcus were cultured from C.S.F. in 14 cases(19.100).
10) 20 cases out of 170 were expired during their hospitalization (mortality: 11.8%).
